How to Print a Temporary Anthem Medical ID card:
(Physical cards are expected to be delivered 7 - 10 business days after January 1st)
For immediate ID card information call BeneSys at 855-696-2909 ext 8604

Already a PHBP participant? Go to anthem.com/ca and log in to your account or create
a new one using your existing Member ID. Even if you are changing medical plans,
your Member ID will stay the same, only your Group number will change.

If you are new to the PHBP as either a Freelance or Staff Employee, call BeneSys at
855-696-2909 or email them at staff@PHBPBenefits.org with your full name and last 4
digits of your SSN and they will provide you with your member ID so you may create an
account and log in per the above instructions.
